The authors describe a case of a severe aortic stenosis patient with concomitant 3-vessel coronary artery disease and severe left ventricle systolic dysfunction who presented in cardiogenic shock. It was decided to perform a balloon aortic valvuloplasty as a life-saving procedure and as a bridge to surgery or transcatheter aortic valve implantation. The patient improved and is alive and well after 3 years. Balloon aortic valvuloplasty is a procedure that can be done in acute setting, improving the hemodynamic status of the patient on the short-term. In current clinical practice, it is rarely performed, due to its transitory results. Nonetheless, it is a safe procedure, currently with better results due to technic improvement, and it is re-emerging in Cardiology internationally. It can be used either as an emergent procedure (changing patient’s prognosis) or as a palliative measure for symptomatic improvement, and it should be considered in these cases.
